The zoledronic acid preclinical and preliminary clinical data favor this new bisphosphonate over clodronate and pamidronate, and intravenous infusion therapy with zoledronic acid appears promising in the prevention of bone metastases and shall therefore be investigated in the present study. This protocol contains a substudy measuring bone mineral density in men with high risk prostate cancer starting with hormone therapy. In a recently completed trial Zmeta Protocol 705 ; , Zometa4 mg given as a 15-minute intravenous infusion every 12 weeks for one year 5 treatments ; significantly increased lumbar spine and hip bone mineral density compared to placebo in 106 men with prostate cancer who were beginning androgen deprivation therapy.44 Men who received the Zomwta infusions had an increase in lumbar spine bone mineral density BMD ; of 5.16% p 0.001 vs. placebo ; . BMD increased by 1.08% in the total hip, 1.64% at the femoral neck, 2.37% at the trochanteric region, and 3.39% at Ward's triangle, and decreased by 2.62%, 1.85%, 2.45%, and 0.58%, respectively, at these sites in the placebo group p 0.001 for difference between zoledronic acid and placebo at the total hip, femoral neck, and trochanteric regions, respectively ; . It is expected that in the present study Zojeta in addition to the prevention of bone metastases will show its potential in preventing hormone therapy induced bone loss and imodium.

Zometa warnings

Zometa has been shown to reduce the risk of bone complications by some 40% over a two year period. It is licensed for prescribing in the UK but has not been assessed by the National Institute for Clinical Excellence NICE ; nor is it on their agenda until after 2005. It costs some 120 per month.
Update Reviews on Zometa, and these had not led to any change in relation to the SPC wording about renal adverse events. In particular, in Section 4.8 the SPC stated: `Adverse reactions to Zoemta are similar to those reported for other bisphosphonates and can be expected to occur in approximately one third of patients' as noted by the Panel in its ruling. Thus Zometa did not appear to have `special concerns' compared to other bisphosphonates. Novartis noted that Roche had again cited the fact that renal toxicity was listed on the SPC as `common', and that Chang et al as well as other publications referred to renal toxicity. Renal toxicity was listed in the SPC as common because it was present in 1-10% of patients in clinical trials, this was reflected in Section 4.8: `There have been some reports of impaired renal function 2.3 % ; , although the aetiology appears to be multifactorial in many cases'. This statement had not been amended with the post-marketing experience of Zometa, and reflected the fact that the patients treated on the clinical trial programme, and subsequently, included some particularly those with multiple myeloma who might have had a multifactorial aetiology to their renal impairment. Chang et al stated nothing different it recommended renal monitoring, adequate hydration and discontinuation of treatment with Zometa if renal deterioration occurred, exactly as in the SPC. The letter from Chang et al was not an official FDA position. Novartis noted that Roche had also reiterated the SPC recommendations about not using Zometa in patients with severe renal impairment and also the use with potentially nephrotoxic agents. Neither of these statements negated the fact that Zometa was generally well-tolerated in a population of patients with advanced cancer. Novartis noted that Roche had introduced a new element to its complaint citing, for the first time, the statement `Adverse reactions to Zometa were comparable to those reported for pamidronate disodium or placebo'. Novartis understood that no new complaint could be introduced at this point in the process, and so it did not comment further on this aspect. FURTHER COMMENTS FROM ROCHE Roche stated that Novartis had incorrectly stated that the Zometa SPC was not subject to any particular warnings. Section 4.4 of the SPC Special warnings and precautions for use ; stated that Zometa `should be withheld if renal function has deteriorated' and `. the use of Zometa is not recommended in patients with severe renal impairment'. This was not the case with Bondronat. Whilst the Zometa SPC might state that `Adverse reactions to Zometa are similar to those reported for other bisphosphonates and can be expected to occur in approximately one-third of patients', this must be taken into context. `Similar' did not mean identical. Bondronat was launched after Zometa in metastatic bone disease and this SPC statement now represented an historical perspective. It did not match the fact that clinically relevant renal toxicity was not a feature of Bondronat. The Bondronat SPC stated `Clinical studies have shown no and meclizine. Were construed to prohibit such membership, however, the inclusion of an individual who was not a member of the IEP team, while a violation of the statute, would not constitute such a procedural violation would not deprive the Student of a FAPE. The federal IDEIA states that: "In matters alleging a procedural violation, a hearing officer may find that a child did not receive a free appropriate public education only if the procedural inadequacies " I ; impeded the child's right to a free appropriate public education; " II ; significantly impeded the parents' opportunity to participate in the decision-making process regarding the provision of a free appropriate public education to the parents' child; or III ; caused a deprivation of educational benefits." 20 U.S.C. 1415 f ; 3 ; E ; The Parent introduced no evidence at the hearing to establish that including an individual on a manifestation determination team who is not a member of the Student's IEP team would impede the Student's right to a FAPE, would significantly impede the Parent's opportunity to participate in the process, or would cause a deprivation of educational benefits. For the reasons stated above, the IHO concludes that the Student was not deprived of a FAPE because a non-IEP team member was included on the manifestation determination team and signed the manifestation form. The School's failure to permit the Parent an equal voice in determining the composition of the manifestation determination team, the failure to include all IEP team members on the manifestation determination team, and the School's failure to provide the required prior written notice to the Parent did, however, deprive the Student of a FAPE. If the School had provided the notice required by Ohio Administrative Code Section 3301-510-05 and Section 1415 of the IDEIA, the Parent would have had notice of the suspension proposed by Mrs. Dewey, the record or report that was the basis for Mrs. Dewey's decision to suspend the Student, the Parent's procedural rights in this setting, the placement of the Student during the pendency of the due process proceeding and the other information required by federal and Ohio law to be included in such notice. Without that notice the Parent could not adequately prepare for or participate in that manifestation determination meeting. Research has shown that treatment with Zoladex, in conjunction with radiation therapy, can prolong survival in advanced stage prostate cancer patients. The purpose of this study was to assess the nutritional effects of a moderately reduced sodium diet enriched with n-3 fatty acids, antioxidants, B vitamins, and certain amino acids cardiac diet ; in dogs with chronic valvular disease CVD ; . Dogs with asymptomatic CVD International Small Animal Cardiac Health Council class 1a or 1b ; were studied. After a baseline cardiac evaluation, blood was collected for a biochemistry profile and for measurement of thiamine, amino acids, vitamins C and E, 8-F2-isoprostanes, nitrite nitrate, and fatty acids. Urine glycosaminoglycans GAGs ; also were analyzed. After baseline measurements in the dogs with CVD were compared to those of healthy control dogs, all dogs with CVD were changed from their usual diets to a run-in diet to ensure that all were eating the same diet before starting the cardiac or placebo diets. After 4 weeks of exclusive feeding of the run-in diet, dogs were re-evaluated and then randomized to receive either the cardiac diet or a placebo diet exclusively in a double-blind fashion. A final evaluation was performed after dogs had eaten the cardiac or placebo diet for 4 weeks. All measurements obtained at baseline were repeated at the 4and 8-week visits. At baseline, there were no significant differences between the CVD n 29 ; and healthy control groups n 12 ; in median age CVD: 8 yrs, range 4-14 yrs; Controls: 9 yrs, range 5-11 yrs ; or median body weight CVD: 8.9 kg, range 5.7-29.5 kg; Controls: 10.1 kg, range 3.9-21.0 kg ; . Dogs with CVD had significantly lower baseline serum sodium and chloride concentrations compared to the healthy controls. A number of plasma amino acids, including methionine, cystathionine, and arginine, also were found to be different between the CVD dogs and healthy controls at baseline. After 4 weeks of exclusively eating either the cardiac n 14 ; or placebo n 15 ; diets, vitamins C and E, cholesterol, and triglyceride concentrations increased significantly more in the cardiac diet group. Thiamine concentrations increased significantly in the cardiac group but not in the placebo group and there was a significant decrease in taurine concentrations for dogs in the placebo group but not in the cardiac diet group. Plasma nitrite nitrate concentrations increased significantly in both diet groups. These results suggest that dietary modification can alter certain circulating antioxidants and amino acids that may be beneficial in dogs with early CVD.
